      <image:caption>Accurate patient weight is essential for administering medication and treatment correctly, yet for immobile patients, hoist scales are often the only option available which can be impractical, stressful, and challenging to use. In such cases, weight may need to be estimated, which can affect medication accuracy. The Patient Transfer Sliding Scale eliminates the need for estimates, allowing immobile patients to be weighed accurately in seconds. With the scale integrated into the transfer board, weighing becomes a seamless part of the patient transfer process, providing greater dignity for patients and reducing lifting for hospital staff.</image:caption>

      <image:title>Past Projects - Pressure Mapper - Early identification of pressure ‘hot spots’ in patients is crucial for both their comfort and overall wellbeing.</image:title>
      <image:caption>Preventing pressure injuries not only improves patient outcomes but also helps reduce healthcare costs. To support this, The Care Foundation has recently funded the purchase of a Pressure Mapper for the Region. In the past, detecting a patient’s pressure points relied on time-consuming, low-tech methods, and a Pressure Mapper was only hired in extreme cases due to the expense. Now, with a Pressure Mapper readily available, clinicians can quickly and accurately identify areas where pressure relief is needed. This allows for optimised seating, bedding, and cushioning solutions, significantly improving patient care both in hospitals and at home.</image:caption>

Scholarships are open to all staff who work for or are contracted by Te Whatu Ora Nelson Marlborough for a minimum of 20 hours per week and provides successful applicants with up to 50% of their study fees.       <image:caption>The Better Living Conditions initiative was created by Age Concern Nelson Tasman to improve support for older adults living in poor and unsanitary conditions by working collaboratively with multiple agencies. These individuals frequently lack support, have limited finances, and face health issues that go unaddressed due to difficulties in accessing healthcare. The Better Living Conditions approach assists these individuals in a consistent and effective manner and prevents double-ups by ensuring everyone around the table is on board with what is happening. The Care Foundation provided grants in 2022, 2024 and 2025 to support the Better Living Conditions initiative.</image:caption>

      <image:caption>As the saying goes “alone we do a little, together we can do so much” and The Care Foundation has certainly seen the benefit of working together with other community groups. Recently in Marlborough the Freemasons (Lodge Eckford 334) approached us looking to collaborate and to support and benefit a vulnerable group in our community. We had already collaborated with The Marlborough Equipment Trust to fund the specialist outdoor play equipment at the Wairau Respite Home but a further need was identified by staff for specialised equipment inside the Home.  To mark its 75th anniversary of service to the Marlborough community, the Lodge jumped at the opportunity to support this initiative by funding a range of specialist sensory equipment - a Sensory Kit, a Protac Sensit Puff Chair and a Squeeze Machine.</image:caption>

      <image:caption>Manu Ora is a charitable organisation based in Blenheim providing a unique service within a Kaupapa Māori model.  Manu Ora was set up by a group of experienced health professionals who were driven by the need to provide all aspects of hauora (wellbeing) for everyone.  Breaking through the inequity barriers, they provide healthcare for high need patients focussing on access and affordability. The Care Foundation were with Manu Ora from their beginnings, funding bariatric and diabetic care equipment along with a medical kit for their outreach home visits.</image:caption>

      <image:title>Past Projects - Nelson Child Development Services Therapy Playground - When Child Development Services moved to a new site on Nelson’s Braemar Campus they were delighted with their new location.</image:title>

The staff could see the extra development opportunities that this space would create as a playground, not only as a treatment, assessment, and therapy space but also as an area to have fun in. Virtual Reality (VR) provides an opportunity to change this by enabling them to experience situations, activities, and environments that they would never otherwise have been able to.       <image:title>Past Projects - Suicide Bereavement Support: WAVES Programme</image:title>
      <image:caption>Suicide is a whole of society issue. The death of a loved one to suicide is life-altering and traumatic for those left behind. Bereavement support is a crucial element in reducing trauma and suicide risk for those bereaved by suicide. Yellow Brick Road runs WAVES, an 8 week free of charge, practical and supportive grief education programme for those affected by suicide.  The Care Foundation supported the work of Yellow Brick Road by funding the delivery of the WAVES programme in Nelson and Marlborough. The programme is delivered by skilled facilitators in small groups providing healing support and promoting a healthier grief process.</image:caption>

      <image:caption>A dawn blessing on 30 March 2023 marked the opening of the brand-new Wairau Children’s Respite Home.  This is a purpose-built house which has been designed to allow for up to six families with children who have disabilities and complex needs to access respite care for their tamariki at any given time. The Care Foundation in partnership with Marlborough Equipment Trust provided funding for the accessible and safe playground at the Respite Home. Government funding had only allowed for a simple grassed finish and basic play equipment, however due to the nature of the tamariki staying at the Home it was everyone’s wish to provide them with a truly accessible space that enriches their experience.</image:caption>

      <image:caption>At the end of 2022 a new purpose-built Haemodialysis Unit opened at Nelson Hospital servicing patients in the Top of the South.    The Care Foundation funded one of the eight specialist chairs and also provided television screens and headphones for each chair.  With patients spending between 4-6 hours in the therapy chairs each visit, this individualised entertainment provides a welcome distraction for patients while they receive treatment.</image:caption>

      <image:caption>Motueka District Nurses provide a range of services to the wider Motueka community which are beyond the scope of general practice but do not require hospital admission. They also work with the Nelson Tasman Hospice to provide a palliative care service in the community. Together with the Hospice they provide equipment to patients to enable them to be treated in their own homes. Often these patients do not qualify for equipment supplied by Te Whatu Ora Nelson Marlborough as they do not meet the criteria and at times there is simply not enough equipment for all patients.</image:caption>

That’s why The Care Foundation chose to support Wellby, a community-led initiative helping the people of Nelson and Tasman to better connect with their communities and improve their wellbeing. Wellby connects people in three ways: through hosted Talking Cafes which provide a welcoming and accessible environment for people to connect with others; by promoting the online Found Directory which contains listings for community groups, clubs and services; and by training people to become “Signposters” who help others find the information they need to enhance their supportive networks.</image:caption>
